1. Aadhaar Number : The Aadhaar card number shall be of the proprietor in the case of a proprietorship firm, of the managing partner in the case of a partnership firm, the owner of the company in the case of private limited, any for the wo partners in case of partnership company and of a karta in the case of a Hindu Undivided Family (HUF). (“यहां 12 अंकों का आधार नंबर भरें।”) 
 
2. Name of Applicant : Put the applicant’s name exactly as it appears on their Aadhaar card. “आवेदक का नाम बिल्कुल वैसा ही डालें जैसा उनके आधार कार्ड पर दिखाई देता है। 
 
3. Social Category :Choose the applicant’s social category from the list of alternatives.
“विकल्पों की सूची में से आवेदक की सामाजिक श्रेणी का चयन करें।”
 
 4. Type of Organization : Select the type of organization from the given options which will get printed on Udhyam Aadhar Certificate.
दिए गए विकल्पों में से संगठन के प्रकार का चयन करें जो उद्यम आधार प्रमाणपत्र पर मुद्रित हो जाएगा।
 
5. Name of Enterprise / Business :Fill the name of Business / name of the Enterprise / name of the company which will get printed on Udhyam Registration Certificate.
“व्यवसाय का नाम / उद्यम का नाम / कंपनी का नाम भरें जो उद्यम पंजीकरण प्रमाणपत्र पर मुद्रित हो जाएगा।”
 
6. Location of Business/Plant : Please correctly enter the location of business, office, plant address with district, pincode and city.
“कृपया व्यवसाय का स्थान, कार्यालय, संयंत्र का पता जिला, पिनकोड और शहर के साथ सही दर्ज करें”
 
7. PAN: Fill 10 Digit PAN Number Number (In case of proprietorship Submit owner’s PAN number, In case of PVT LTD company submit company pan no. In case of partnership company any ones pan card number ) “10 अंकों का पैन नंबर भरें (स्वामित्व के मामले में मालिक का पैन नंबर जमा करें, प्राइवेट लिमिटेड कंपनी के मामले में कंपनी पैन नंबर जमा करें। साझेदारी कंपनी के मामले में कोई भी पैन कार्ड नंबर)”
 
8. Date of Incorporation / Registration : Select the date of  commencement of the business. “व्यवसाय पंजीकरणकी की तारीख का उल्लेख यहां करें।”
 
9. Mobile No : Please enter the applicant’s correct mobile number. On this phone number, a udhyam aadhar confirmation code will be sent.

 10. E-Mail ID : Enter the applicant’s correct mail ID here.
